To get the Stealth Camouflage, choose to submit to Revolver Ocelot’s torture. This will lead to an ending in which you escape with Otacan. As a result, Otacan will give you the Stealth Camouflage, which will be usable on your next playthrough.

How to Use Stealth Camouflage

Makes Snake invisible

Equipping the Stealth Camouflage makes Snake completely invisible, allowing you to pass through enemies and surveillance cameras without being detected.

Wolves, on the other hand, can detect your scent, which means the effect of the stealth camouflage is ineffective against them.
