To get a Tormented Invoker of Varshan in Diablo 4 Season of the Malignant, you must defeat the seasonal boss Echo of Varshan in the secret room of Malignant Tunnels on World Tier 3. Doing so rewards you with the recipe to craft the Tormented Invoker of Varshan at Cormond’s Wagon.
Only becomes available after clearing the Season 1 story campaign.
Before you are able to challenge the World Tier 3 variant of Echo of Varshan, you must first clear the Season 1 story campaign quests to obtain the regular Invoker of Varshan. This item gives you access to the secret room inside Malignant Tunnels to challenge the base version of the boss on World Tier 1 or World Tier 2. Defeat the boss to get the Foul Invoker of Varshan recipe, as well as Malignant Hearts.
Once you have acquired the Foul of Invoker of Varshan recipe, head to Cormond’s Wagon to craft the item and use it inside the Malignant Tunnel’s secret room on World Tier 3. Then, defeat the World Tier 3 Echo of Varshan to get the Tormented Invoker of Varshan crafting recipe.

The Tormented Invoker of Varshan is a special item used to summon and battle the strongest variant of Echo of Varshan inside Malignant Tunnels on World Tier 4. This limited boss will normally yield the rarest versions of Malignant Hearts (with a chance of any of them being a Wrathful Malignant Heart) upon defeat.
Farming the Echo of Varshan is among the best ways to get Malignant Hearts during the end-game stages. More powerful variants of the boss encountered on higher World Tiers will drop rarer versions of Malignant Hearts, making it worthwhile to challenge them once your Seasonal Character is well-equipped and at a significantly high level.
