While not confirmed from any official material, it is expected that Star Ocean: The Divine Force‘s Chaos difficulty is unlocked once you beat the game on Universe (Hard). More information on this will be added when the game is available.
About Chaos Difficulty
Chaos difficulty is essentially the game’s Very Hard mode and presents the greatest challenge in overall level of combat against various enemies throughout the main story. Note that getting 100% completion (Platinum trophy) requires you to obtain the Master of Chaos achievement (beat the game on Chaos level).

Trophies and Achievements List
for the Master of Chaos achievement, you only need to beat the last boss on Chaos Difficulty.
Craft the Postgame-amour/weapon and than change the difficulty in the set up menu on Hard, beat the last boss (to unlook Chaos Difficulty). Save the game. Start a new game on Chaos Difficulty. Play until you beat the tutorial monster. Save the game. load the game from befor the last boss an you will see the game is in Chaos Difficulty.
Now directly to the last boss and beat him again an you will get the Master of Chaos achievement without play the complet game on Chaos Difficulty.
