How to Get the Best Accessory (Omnipotent Orb)
Reward for clearing Request #55 (Defeat Elizabeth).
The Omnipotent Orb can be obtained as a reward for clearing Request #55 in Tartarus which requires you to defeat the superboss Elizabeth. She is an extremely challenging boss so it is recommended to raise your party to as high as level 99 in addition to equipping end-game gear and Personas before attempting to fight her.

You can also get the Omnipotent Orb as an extremely rare Heart Item from Messiah. The steps to get the accessory via this method are listed below.

3 | Level up Messiah just before it hits 98 and register in the Velvet Room book. |

7 | If you got Omnipotent Orb, draw another Messiah from the Velvet Room book and repeat steps 5 to 6 until you get as many as you need. |
Raise the Judgement Social Link to max to unlock Messiah in Persona fusion.
To unlock Messiah for special Persona fusion, you must reach Floor 254 of Tartarus and max out the Judgement Social Link. Afterwards, you will be able to create Messiah by fusing various end-game Personas.
Persona 3 Reload Judgement Social Link Guide
Obtain Messiah through special Persona fusion.
Messiah is obtained by fusing Orpheus and Thanatos, the latter of which require several Personas itself to acquire. This step requires a lot of patience as you are going to perform special fusions Alice and Thanatos at least three (3) times.

Level up Messiah just before it hits 98 and register in the Velvet Room books.
Once you obtain Messiah, raise its level just before 98 (just before it yields its Heart Item). Before proceeding, register Messiah at the Velvet Room book. This saves you the time of having to level it up again later to the level where it will give its Heart Item, allowing you get the best accessory in the game faster.
Persona 3 Reload Heart Items List and Guide
Before getting Messiah to level 98, save your game. This way, you can just reload if you do not receive the Omnipotent Orb as a Heart Item from Messiah.
Level up Messiah to 98 to get the Heart Item from it via Conception.
Raise Messiah’s level to 98 to make it yield its Heart Item which has a 1% chance of being the Omnipotent Orb and a 99% chance of being one of the strongest armor pieces (body armor and shoes/legs) for human characters, Aigis, and Koromaru.
Persona 3 Reload Best Armor for Each Character
Discard the current Messiah and draw it again from the Velvet Room book.
Once you get the Omnipotent Orb, discard the current Messiah and draw another one from the Velvet Room book. Then, level it up again to 98 to make the new Messiah yield another copy of the accessory. Repeat the process until you get enough copies of Omnipotent Orb that you need.
