In Shin Megami Tensei V: Vengeance (SMT 5: Vengeance), fusion accidents are rare events during demon fusion that cause the resulting demon formed to be up to 7 levels higher than the player character (Nahobino). This occurrence is indicated by the screen turning red when fusing demons in the World of Shadows.

How to Trigger Fusion Accidents
Higher chance to occur during a Full Moon.
Fusion accidents are more likely to occur during a Full Moon phase than at any other time in the game. This means that you should try to fuse demons at this time if you intend to obtain one who is at a higher level than the main character. Otherwise, avoid fusing demons when the moon is full.

For forcing fusion accidents to happen, try saving your game before going into the World of Shadows. Then, when the moon phase is at Full, begin demon fusion to trigger the accident. Repeat the process by simply reloading your save until the fusion accident occurs.
